Here are a few examples of forms we've seen on our clients' sites. 

Example 1

This form simplifies the process for someone trying to register multiple guests for a convention.

  • Instead of a having to make an entry for each guest who needs to be registered, a single box is used for listing every guest.
  • Right below there is a drop down box to select the number of guests registered, enabling one easy payment. 
  • This form is unique for allowing people to submit their registration and mail in a check.

    This client used a form to create a job application.

    This form makes creative use of one of our commonly used features in our forms module.

  • In the middle column they use page text form items to add extra spacing above & below each field. This is done by adding line breaks in the page text item

    • Example 4

    Here we have a simple facility rental form. 

    • The client has areas of the facility available, so the checkboxes to the left of the different rental options really come in handy. We call these checkboxes Radio Buttons.
